• It is our goal to make this a communication tool for your use.  We will do our best to keep it current.
• All ill students need to go through the nurses office to be released for the day.
• Current Immunization Information for 2008-2009 School Year can be found at the Arizona Department of Health Service , Immunization Office, (602)364-3632.  Students who are non-compliant with state guidelines will not be allowed to register at Chaparral High School. Scottsdale Health Care offers free immunizations every month. You can access a schedule of clinics under Health Links/Immunizations/Clinics. You can fax immunization records to our office at 480 484-6545.
• Emergency Health Cards need to be presented yearly at registration in order for the registration process to continue.  Those students who do not have  an emergency health card will be not allowed to register until one is turned in.
• If your child needs to take any medication during the school day, parents/guardians must complete and sign a Medication Administration Record permission form (Medication Permission Form). All medication must be in the original pharmacy bottle with the dosage and instructions clearly written on the prescription label.
• There has been a change in Scottsdale Unified School District guidelines regarding the over-the-counter medication that is given out in our school Health Centers. Previously, the consent to give medication was on the back of your student's emergency card. It gave our school nurses the option of administering a limited amount of over-the counter medications (like Tylenol, Ibuprofen, Tums, Antibacterial Ointment, Calydryl etc.). This has changed. We now need you to complete a Medication Administration Record permission form (Medication Permission Form). Also, you must provide the specific, age-appropriate medication (as listed on the medication label) and it must be in an original, sealed container before any over-the-counter medication will be given to your child. Medication Permission Forms can be downloaded from this website under our Medication Policy and submitted to the Health Office. We are sorry for any inconvenience this may cause you or your student, but this new guideline is in the best (and safest) interest of all our students. Please feel free to call the school nurse or stop by the Health Center for more information or to provide the proper medication and documentation for your student.
